Morphine Milligram Equivalents (MME) Calculator

This calculator utilizes the most recent CDC Clinical Practice Guideline for Prescribing Opioids for Pain published in 2022, to calculate the Morphine Equivalent Dose (MED), also known as Morphine milligram equivalent (MME) dose, for patients who are taking one or more opioid medications.

With the exception of Fentanyl transdermal, the calculator assumes that the dose entered is the daily dose for the patient. However, you can have the calculator calculate the daily dose if you know how often the drug is being administered. For example, if hydrocodone 5mg/acetaminophen 325mg is administered every four hours as needed for pain, you would enter 5 * 4h and the calculator will calculate a daily dose of 30 mg/day.

It is important to note that this calculator should not be used to determine doses when converting a patient from one opioid to another. Equianalgesic dose ratios are only approximations and do not account for genetic factors, incomplete cross-tolerance, and pharmacokinetics.

When entering combination drugs such as acetaminophen and hydrocodone, only the dose of the opioid component should be entered, which in this case would be hydrocodone.

This calculator should not be used in pediatric patients, due to unpredictable rates of absorption and the risk of overdose.
